First-Time Patients
At your first visit, you will be greeted by our friendly front office staff and asked to complete any necessary paperwork. If you would like to complete paperwork prior to your appointment, click the “Download” button below.
Once your New Patient paperwork is completed, one of our amazing dental hygienists or dental assistants will greet you and guide you into an X-ray room. Taking dental X-rays will help us evaluate any problems with your teeth that are invisible to the naked eye.
Once your X-rays are completed, your dental hygienist or assistant will ask you about the reason for your visit, any dental concerns you might have, and any goals you may have as it pertains to your oral health. He or she will also review your health and dental history.
If you are visiting to investigate cosmetic or restorative dentistry, you will then meet with one of our South Park dentists to outline a treatment plan. If your visit is for a routine cleaning and checkup, your hygienist will perform the cleaning, and D. Smith or Dr. Hendrick will inspect your teeth for any signs of damage or decay.
Tips for Your Appointment
ARRIVE EARLY
Please arrive at least 10 minutes before your scheduled appointment time. This allows our staff adequate time to review your information and make other preparations. If you need to complete new patient paperwork, plan to arrive at least 20 minutes before your appointment.
BRING ALL NECESSARY DOCUMENTS
Remember to bring your current insurance card; a list of medications, your driver’s license; and any dental records you would like to transfer, such as X-rays. Don’t forget your payment method– our office accepts cash, Visa, MasterCard, American Express, Discover, and CareCredit.
GIVE A 24-HOUR CANCELLATION NOTICE
If you are not able attend your appointment, we require a 24-hour notice of cancellation. This enables us to accommodate others with dental needs and/or pain. Patients who do not give proper cancellation notice may be subject to a cancellation fee.
SCHEDULE YOUR NEXT VISIT
At the end of your appointment, your dentist will share your treatment plan with the front office so that our staff can review any insurance and billing questions with you. We will then schedule your next appointment so that you are all prepared for your next visit.
